Symptoms
When using WebCenter 16.0 and convert a Simple Approval to Staged Approval, user is presented the Approval Stage Name field but the user is not forced to Add a Stage 2.
While v16.1.x Approval setup may be more intuitive, it does not allow for this anymore.
Project Manager allowed to Name the Approval can be important where users are not in Groups, and/or Project Approvals do not use descriptive Role Names to illustrate (at a glance) the context of the Approval.

Installation Instructions
Unzip the hotfix zip file.
Navigate to <drive>:\Esko\Artios\WebCenter\WebServer\tomcat\webapps.
Overwrite the contents of wcr-static with the wcr-static folder of the zip.
Overwrite the contents of desired webcenter instance folder with WebInstance folder of the zip.
